Contract for the Carrying out of Certification Work - Subdivision pursuant to the Environment Planning and Assessment Act 1979 & the Building and Development Certifiers Act 2018. 

A copy of this form will need to accompany your application for a Subdivision Works Certificate (SWC) or Complying Development Certificate (CDC).  Please include this in your attachments on the NSW Planning Portal when applying for your Construction Certificate. 

This form can only be submitted via the NSW Planning Portal when you apply for your SWC or CDC Application.

This forms needs to be submitted as an attachment as part of your Subdivision Works Certificate Application via the NSW Planning Portal.
