This form should be completed when requesting a new or changes to a domestic waste collection service for properties (e.g. houses, free-standing dwellings etc.) that are NOT under strata title.
This form has 2 sections:
SECTION 1 - New Domestic Waste Service Request – complete pages 1,2 & 4
SECTION 2 - Change To Existing Domestic Waste Service Request – complete pages 1,3 & 4.
This form must be completed by the owner of the property or an authorised agent. Owners Authority and/or Directors authority must be provided with this application if not already provided to Council.
